In other words, your problem is most likely to be the cable between the phone and switch (including any patch cables). No need to change cables in the rest of the configuration to make it work. Power will only matter between your switch (or injector) and the phone. The cable I'm using in that case is the extension cable that came with the phone, which would normally go from the wall jack to the phone. However, I can connect the phone directly to the port on the switch and the phone powers up and works.
Using that setup, the phone does not power up. On the telephone end, I connected the blue/blue-white pair to the two middle prongs of a female RJ11 connector. Not sure exactly what model of phone you're using, but yes CAT5e/6 should be fine. The reason for this is you need a pair to pass power, and a pair to pass data. Both of those cables you'll need to make identical with two pairs. So really what you're doing is making 2x RJ45 > RJ11 cables and plugging one from the phone server to the patch, and the other from the walljack to the phone.
Since your server is the element providing power then all links in the chain need to be capable of carrying power. do you see 4 wires? That'll be how it passes power.
Take a close look at that cable and look through the clear plastic RJ11 head itself. So in your previous post when you plugged directly in to the phone server with a premade RJ11 M/M cable it worked. Sorry to be so clueless about all of this. I did try using a second pair, just to see if that made any difference, but still not working. So it seems to be Phone server (RJ11) > Patch/WallJack (RJ45) > Phone itself (RJ11). There's no switch in between the phone server and the walljacks that I can see, but there are Cat5/6 cables connected directly to the phone server so it looks to me like there is a direct connection between each walljack and the phone server. I can set up the virtual voicemail box easily, but I can't for the life of me figure out how to tell the system to route the call to it if it isn't answered. The way we want it set up is to have all stations ring when there is an incoming call (which I have set up by adding all stations to the CO ring assignment,) and then if no station picks up we want the call to go to a virtual voicemail box. I managed to find a copy of the PCAdmin program to connect to it from a computer, and I've got everything back the way it should be in the settings, except for the voicemail. I'm trying to get it back up and running while we research a newer phone system. Due to a power surge or something, it lost all of it's settings and reverted to factory default, and the company that installed it (which was long before I worked here) doesn't provide support for it anymore. I am currently trying to fix our office's phone system it's a Vertical SBX IP 320 which was installed in 2011.
